Patient information

Everything you need before your first visit. Filling the paperwork out at home saves about fifteen minutes in the waiting room.

Before your visit

New patient paperwork

Please print your paperwork and bring it completed to your appointment. If your insurance requires an authorization, have that information sent to our office ahead of time or bring it with you, along with any medical records you have.

Thank you — we look forward to meeting you. Contact us with any questions.

Upload the PDF before publishing Put your paperwork file at forms/new-patient-paperwork.pdf, or change the button link above to wherever you host it. Until then this button leads nowhere.

Bring to every appointment

  • Insurance card
  • Photo ID
  • Current medication list, including doses
  • Any prior records, imaging or test results
  • Authorization, if your plan requires one

Running late or need to reschedule?

Call (321) 321-3980 as early as you can. Giving up a slot you cannot use lets us offer it to someone waiting.

Optional — patient portal link If you want patients to request appointments or message the office online, link your eCW patient portal or healow booking page here rather than adding a web form. Requests then land directly in your schedule, and no patient information passes through the website.

Coverage

Most insurance plans are accepted. Some plans require a referral from your primary care physician before a specialist visit, and some vein procedures require prior authorization. Call the office and we will check your specific plan before you book, so there are no surprises afterwards.

List your accepted carriers here Naming the plans you accept — Medicare, and the specific commercial carriers — is the highest-value content you can add to this site. It is the first thing a prospective patient checks and the most common reason they call a different practice instead.

Questions before your visit?

Our staff can check your coverage and answer questions by phone.
