New patient paperwork
Please print your paperwork and bring it completed to your appointment. If your insurance requires an authorization, have that information sent to our office ahead of time or bring it with you, along with any medical records you have.

Bring to every appointment
- Insurance card
- Photo ID
- Current medication list, including doses
- Any prior records, imaging or test results
- Authorization, if your plan requires one
Running late or need to reschedule?
Call (321) 321-3980 as early as you can. Giving up a slot you cannot use lets us offer it to someone waiting.
Coverage
Most insurance plans are accepted. Some plans require a referral from your primary care physician before a specialist visit, and some vein procedures require prior authorization. Call the office and we will check your specific plan before you book, so there are no surprises afterwards.
